The Wound Care Nurse serves as the clinical leader for the facility's wound care and skin integrity program, supporting both short-term rehabilitation and long-term care residents. In this role, you will oversee the prevention, assessment, treatment, and healing of wounds while promoting resident-centered care and ensuring compliance with regulatory and quality standards. As the wound care expert, you will collaborate with interdisciplinary teams to develop and implement individualized treatment plans that optimize healing outcomes and enhance residents' quality of life. Key responsibilities include conducting comprehensive skin and wound assessments, performing wound measurements, documenting and monitoring healing progress, and entering and managing wound care orders within the electronic medical record. The Wound Care Nurse works closely with physicians, nurse practitioners, rehabilitation staff, dietitians, and nursing teams to coordinate care and drive positive clinical outcomes. This position also plays a critical role in pressure injury prevention, quality improvement initiatives, staff education, and competency development. The ideal candidate is a skilled clinician with strong assessment and communication abilities, sound clinical judgment, and a passion for evidence-based practice, continuous improvement, and exceptional resident care.
