Position Title:
Resource Specialist
Work Location: Remote - Region B, Marion, Lake, Sumter, Hernando, Alachua & Gilcrest County."
(There may be opportunities to attend community events or events at one of our Community Connections Center, but we are not doing home visits as of this time nor have we discussed it for any time soon. Almost primarily remote.)
Assignment Duration: 3 Months, Possibility to extend or convert
Work Schedule: 8am-5pm, Monday through Friday / Weekend work may be needed during emergency situations.
Responsibilities:
- Provides resource support to members and connects members to local resources for various services (e.g., employment, housing, independent living, justice, foster care) based on care/service assessment and plans. Works with leadership to identify the best, most appropriate local resources for member's needs based on industry best practices.
- This position will be remote and will provide care management to assist with coordination of care, including assessing for health care needs, referring to health plan providers, providing education on benefits/expanded benefits, and provide community resources to address social determinants of health needs. The caseload is 100 members. This position will document in an electronic health record and will be required to document outreach notes, complete assessments, and complete a care plan on each member.
Describe the performance expectations/metrics for this individual and their team:
* Compliance with care management policies/procedures including outreach and documentation expectations.
* Individuals will be audited on a monthly basis to ensure charts are in compliance.
Tell me about what their first day looks like:
* The first day will align with training/onboarding. The first day will consist of setting up computer, completing onboarding tasks, reviewing training schedule, and meeting with the training team. Onboarding is 4 weeks.
