Care Management Program Coordinator (5393)

• Lead, support, and develop a team of six to eight Care Managers through in-person and remote supervision.
• Ensure services meet Together for Youth, Health Home Serving Children, and New York State Department of Health requirements.
• Coach staff using strength-based, person-centered, and solution-focused practices.
• Review case files and monitor documentation to maintain program quality, regulatory compliance, and appropriate service delivery.
• Assign and monitor required Care Manager training and support continued staff readiness.
• Carry a small to moderate caseload while modeling effective care management practices.
• Build relationships with schools, mental health providers, managed care organizations, OCFS partners, and other community resources.
• Develop referral relationships that connect more eligible children and families with needed services.
• Represent the program on agency and community committees and serve as a liaison to external partners.
• Partner with the Director of Care Management to maintain the integrity and effectiveness of the program.
• Monitor referral volume, discharges, billing activity, and other program factors that support fiscal stability.
• Participate in a rotating schedule providing administrative oversight by phone.
• Perform other duties as assigned.

• Three to five years of direct experience in children's Health Home care management is required.
• Bachelor's degree in social work or a related human services discipline is required. A master's degree is preferred.
• Two or more years of supervisory or management experience is highly desirable.
• Demonstrated ability to lead from a solution-focused perspective and support staff development.
• Strong understanding of strength-based and person-centered care.
• Ability to balance staff leadership, program compliance, referral development, and operational priorities.
• Valid driver's license, acceptable driving record, reliable vehicle, and appropriate automobile insurance.
• Ability to travel regionally approximately 20% of the time.
• Ability to work a flexible schedule, including occasional nights and weekends as program needs require.

$60,000-$64,500 annually, commensurate with experience.
