Responsibilities
LifeWorks is seeking a compassionate, organized, and proactive School Social Worker to provide systemwide support for students, families, and school teams. This position will serve as the primary liaison between home, school, community providers, and outpatient services to ensure students receive the support they need to be successful both academically and emotionally. The ideal candidate would have experience coordinating community resources, supporting families through complex systems, improving student attendance, and collaborating with multidisciplinary teams to reduce barriers to student success. Essential Responsibilities:
- Clinical Coordination & Family Support
- Coordinate clinical support services and outpatient referrals for students and families.
- Serve as a consistent point of contact for families while students are receiving a higher level of care.
- Assist families in navigating insurance benefits, accessing community resources, and understanding available behavioral health services.
- Coordinate communication among families, treatment providers, hospitals, and school staff during transitions of care.
- Attendance & School Engagement
- Lead attendance intervention efforts across assigned schools.
- Monitor attendance trends and identify students requiring intervention.
- Coordinate attendance communications with Local Education Agencies (LEAs).
- Complete attendance letters and required documentation.
- Conduct home visits and family outreach to improve student attendance and engagement.
- Coordinate Student Attendance Improvement Plan meetings.
- Student Services & Collaboration
- Patriciate as an active member of Child Study Teams.
- Assist with development and implementation of intervention plans for students experiencing academic, behavioral, attendance, or social-emotional concerns.
- Collaborate with counselors, administrators, teachers, nurses, and related service providers to support student success.
- Facilitate student groups and psychoeducational groups when appropriate
- Crisis Response
- Assist with crisis management and student stabilization.
- Provide support during student behavioral or mental health crises as needed.
Qualifications
Qualifications:
- Master's degree in social work (MSW) required.
- Current state licensure (LSW or LCSW preferred, depending on state requirements).
- Experience working with children, adolescents, and families.
- Knowledge of behavioral health systems, community resources, and special education processes.
- Experience conducting home visits, family engagement, and case management preferred.
- Strong communication, collaboration, and organizational skills.
