Summary:

  • The Lead Inpatient Rehabilitation Therapist provides quality, comprehensive, patient centered physical therapy services including patient assessment, development and administration of a treatment plan and individualized patient goals, documentation, and multidisciplinary communication. The Lead also assists in inpatient medical rehabilitation department program development, team competency and educational development in accordance with accreditation and professional standards, policy and procedure review and development, team and patient scheduling, and team leadership. The role is responsible for mentoring Therapists, Assistants, Technicians, and Students. Collaborates with other healthcare team members and physicians in a multi-disciplinary fashion to deliver the highest quality of care. Participates in departmental and organizational CQI activities. Facilitates the provision of excellent customer service.

Responsibilities:

  • 1. Provides patient care assessment and treatment.

    a. Perform objective initial evaluation.

    b. Assess patients' for the purpose of determining their deficits.

    c. Evaluate patient's physical and mental abilities and analyze data to determine realistic rehabilitation plan of care.

    d. Review regularly physician's referral and patient's medical records to help determine diagnosis and physical therapy treatment required.

    e. Record prognosis, treatment, response, and progress in patient's medical record.

    f. Prepare accurate and timely documentation (e.g. evaluations, plans of care, progress notes, correspondence with physicians or other agencies related to patient care, etc.).

    g. Maintain up-to-date cumulative therapy records.

    h. Facilitate patient skills including use of safe patient handling equipment.

    i. Manage equipment utilized in treatments using appropriate safe patient handling techniques and/or equipment.

    j. Adapt and modify environments, equipment, and materials to assist amelioration of patient's deficit and promote activities that will help patients learn skills within limits of their mental and physical capabilities.

    k. Facilitate social adjustment and prevent physical or mental disability.

    l. Make recommendations for appropriate referrals, resources, and assistive devices.

    m. Demonstrate exceptional clinical assessment and treatment skills in all clinical areas and act as resource for other staff.

    n. Set quantifiable and realistic goals.

    o. Utilize advanced judgment and skills in implementing treatment plans.

    p. Physician communication is timely and documentation is up-to-date.

    q. Facilitate excellent customer service.

    r. Take the initiative to resolve client concerns or confirm that the appropriate party resolves the concerns.

    s. Instruct patients, educational personnel, and family to facilitate skills in areas of physical functioning as well as health maintenance.

    t. Educate patients and family in treatment procedures to be continued at home.

    u. Train caregivers and patients how to provide for the needs of patients during and after surgery.

    v. Model appropriate communication and behavior. Monitor and holds co-workers accountable. Follow Lenoir Memorial Hospital Code of Conduct.

    a. Program Development

    a. Participate in performance improvement activities, CQI activities, perform audits as assigned, tracks program metrics for quality improvement. Utilize organizational performance improvement methodology.

    b. Support CARF standards,

    c. Review and develop policies and procedures for the department.

    d. Maintain clinical competencies and demonstrate clinical leadership through program development, refinement, and management on unit.

    e. Integrate current research and practice guidelines into clinical care.

    f. Seek opportunities for professional growth.

    g. Participate in continuing education for professional development to ensure techniques are consistent with best practice.

    h. Demonstrate ability to provide leadership to other staff in all aspects of care including, but not limited to treatment of patients, customer service, time management, etc.

    i. Arrange the provision of education, and training for new equipment and procedures.

    j. Builds positive relationships with physicians, other healthcare team members and the community.

    3. Teamwork

      1. Provide direction, supervise therapists, therapist assistants, techs, and other staff as assigned Document supervision as appropriate.
      2. Plan, coordinate, and provide training to family, patient, medical professionals.
      3. Develop clinical educational opportunities for students and team members.
      4. Assist peer therapists, therapist assistants, and other team members with workload as needed.
      5. Consult with rehabilitation team to select activity programs and coordinate therapy with other therapeutic activities.
      6. Communicates hospital and departmental positions and information.
      7. Oversee multi-disciplinary clinical care.
      8. Orient new employees and contribute to development of orientation materials.
      9. Assist departmental leadership in Performance Development of team members.

    j. Provide support and coaching to team members to ensure the needs of patients are met with high quality care and positive attitude.

      1. Confer with patient, medical practitioners and appropriate others to plan, implement and assess the intervention program.

    4. Mentoring

    a. Proactively shares clinical expertise.

    b. Delegates duties and provides feedback on performance.

    c. Aids in recruiting and interviewing staff.

Other information:

  • EDUCATION

o Bachelor's or Master's Degree in Physical Therapy from an accredited School of Physical Therapy or Bachelor's or Master's Degree in Occupational Therapy from an accredited School of Occupational Therapy or Bachelor's or Master's Degree in SLP from an accredited School.

  • EXPERIENCE
    • Minimal of 3 years of clinical experience preferred.
  • LICENSURE/REGISTRATION/CERTIFICATION
    • Current NC Board of Physical Therapy license, NC Board of Occupational Therapy license, SLP
  • OTHER SKILLS AND QUALIFICATIONS
    • Analytical abilities to delegate personnel, problem solve with positive solutions. Good interpersonal and communication skills required to facilitate interactions with stated customers and staff members.
  • WORK ENVIRONMENT
    • Exposure to bloodborne pathogens, temperature changes and occasional noxious odors. Stressful work periods occasionally encountered.
  • PHYSICAL REQUIREMENTS
    • Standing for most of the day lifting/assisting patients with transfers, bed mobility and ambulation as well as their exercise programs.
