This position will collaborate with other professionals to evaluate patients' psychiatric condition and to assess client needs as part of the interdisciplinary team. The per diem social worker works collaboratively with all healthcare team members to develop and implement treatment plans that support patient-centered plans of care for both individual patients and the medical community, and assesses and arranges discharge planning needs and post hospital follow up disposition.
Responsibilities include: -Individual psychiatric evaluations -Daily individual contact with assigned patients. - Leading one or more weekly social work treatment groups (as needed) -Family outreach and counseling and psychoeducation as indicated -Coordination with outpatient treatment team -Assesses and arranges discharge planning needs and follow up as appropriate. -Timely completion of Treatment Plan and Discharge Summary that will satisfy clinical and regulatory requirements and arranging referrals related to aftercare as needed. -Collaborates with and provides social work consultation to other disciplines within the setting. -Participates on Departmental and Hospital committees as appropriate. -Other duties as assigned.
