Registered Nurse Care Coordinator (RNCC) - UMA Clinic

As a Registered Nurse Care Coordinator in the UMA Clinic, you will play an integral role inTransitional Care Management and Chronic Care Management for an internal medicine patient population.You will join a team committed to learning, professional growth, and expanding the clinic's Chronic Care Management program, with leadership that encourages nurses to practice with autonomy.

The position offers flexible scheduling options, including five 8-hour days or four 9-hour days plus one 4-hour day. Following approximately six months of orientation, the role may also offer one to two remote workdays per week, based on readiness and operational needs.

The Role

This role is well suited for an experienced RN who understands the complexity of primary care and values the opportunity to build continuity with patients. Working within an Internal Medicine Resident Clinic, you will help patients navigate transitions in care and manage ongoing health needs through organized, responsive, and clinically informed coordination.

Your work will support safe, connected care while contributing to the continued growth of Chronic Care Management at UMA Clinic.

• Manage and support Transitional Care Management and Chronic Care Management activities.

• Coordinate care for patients with ongoing and evolving healthcare needs.

• Use nursing judgment to identify priorities, support continuity, and help patients remain engaged in their care.

• Collaborate within an academic internal medicine environment that brings together nurses, physicians, residents, and other members of the care team.

• Contribute to the expansion of the Chronic Care Management program at UMA Clinic.

• Practice with meaningful autonomy while remaining connected to a supportive clinical team and manager.

• Build trusting relationships with patients through clear communication, follow-through, and compassionate care coordination.

MINIMUM REQUIREMENTS:

Education: Bachelor of Science in Nursing from an accredited nursing program.

Experience: 1 year of registered nursing experience required, 3 years of related and relevant registered nursing experience strongly preferred.

Specialty Board Certification strongly preferred

License/Certification: Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia. American Heart Association (AHA) Health Care Provider BLS certification is required.

PHYSICAL DEMANDS

Job requires standing for prolonged periods, frequently traveling, and bending/stooping. Proficient communicative, auditory, and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 20 - 50lbs. May be exposed to chemicals, blood/body fluids, and infectious diseases.
