The Department of Family Medicine seeks a Licensed Clinical Social Worker or Licensed Professional Counselor to join their outpatient clinic team as an integrated provider on the primary care team.
The provider operates as an integrated team member seeing patients through same-day warm handoffs and scheduled brief consultations. Typical patient contact includes 1-5 visits with a focus on empowering patients for self-management. The role encompasses consultation-based behavioral health services in collaboration with primary care providers across UVA Family Medicine clinics in Charlottesville City, Albemarle County, and Nelson County, with location flexible to candidate preference. The Family Stress Clinic is the Department of Family Medicine's integrated behavioral health program, providing clinical services in support of the department's seven Family Medicine clinics. The program is recruiting for two providers to join the existing team of three full-time providers; the program also hosts a training rotation for three doctoral psychology students to provide clinical services part-time. The department's behavioral health providers regularly provide peer consultation to each other and assist each other with maintaining their primary care behavioral health-specific competencies. Preferred Qualifications *Experience working with a lifespan population *Experience working in interdisciplinary or integrated health care settings. *Demonstrated comfort with interventions appropriate for focused contacts (e.g., Motivational Interviewing, Solution-Focused Brief Therapy, Cognitive-Behavioral Therapy, Acceptance and Commitment Therapy) *Experience managing clinical flow in time-limited settings, including rapid assessment and intervention *Proficiency with electronic health records and integrated documentation *Experience: Minimum of 1-year relevant experience Background checks and pre-employment health screenings will be conducted on all new hires prior to employment. All safety-sensitive positions with direct patient interaction also require a drug screen. Administration: Provides clinical supervision,and trainsnew staff, students, and interns as assigned. Participates in education and training for medical students, residents, and fellows through teachings/didactics, consultation, and clinical observation. Professional: Documentspatient carein accordance withUVAUniversity Medical Centerpolicies and regulatory requirements. Facilitates staff development, in-service training, and educational opportunities to support professional growth and evidence-based practice. Clinical: Providesevidence-based, person-centered clinical therapeutic interventions around a broad range of behavioral health conditions. Completesbio-psychosocialassessments,diagnosis, and development ofculturallycompetent individualizedtreatment plans for patients in both individual and group settings. Participates in treatment planning, care coordination, andadvocatesfor patient preferences within the primary care team, when appropriate. Collaborate with patients (andcarepartners,whenappropriate)to create,followandmaintainprogress on treatment plans. Leadership: Assistswith treatment programdevelopmentand engagesin research activities. Adapts to evolving needs by taking onadditionalresponsibilities, asindicated. MINIMUM REQUIREMENTS Education: Master's degree in Social Work from a CSWE accredited social work program required Experience: 3 years' experience post master's required. Licensure: Licensed to practice as a Licensed Clinical Social Worker in the Commonwealth of Virginia (LCSW) or applicants with an active license from another state must obtain Virginia LCSW within 18 months of hire date. PHYSICAL DEMANDS Job requires sitting for prolonged periods, standing/traveling or use of assistive and climbing (stairs, steps). Proficient communicative, auditory and visual skills; Attention to detail, hear, speak, see, distinguish colors, read, ability to write legibly; Ability to lift/push/pull less than 20lbs. May be exposed to chemicals, blood/body fluids and infectious disease. The starting base rate for this role is $61,443.29 annually.
