At Healthcare in Action, we serve patients experiencing homelessness through an innovative "street medicine" approach by contracting with Medicare and Medicaid health plans as well as hospitals and health systems.

Street medicine entails using a medically equipped van to deliver medical care directly to people experiencing homelessness in locations such as parking lots, encampments, and parks.

**This is a temporary opportunity for a limited period of time**

The Job

The Referral Coordinator plays a crucial role in ensuring the smooth operation of various tasks related to patient data management, communication, tracking, and specialty referrals. The role involves meticulous data entry, efficient response to team inquiries, proactive tracking and monitoring, and active involvement in special projects as assigned.

You Will

  • Create new patient charts and enter relevant information into designated platforms (Smartsheets, Canvas).

  • Review and update patient eligibility information.

  • Conduct audits on patient charts to ensure completeness and accuracy of enrollment forms.

  • Effectively respond to team requests and inquiries on multiple communication platforms (Teams,TigerConnect, Email).

  • Process medical record requests promptly and accurately.

  • Manage incoming faxes, assigning them appropriately to patient e-charts.

  • Monitorvarious communication platforms to ensuretimelyandappropriate staffresponses.

  • Co-support the tracking of Cell Phone Distribution and Phone Line Reassignment.

  • Activelyparticipatein team meetings, contributing insights, and followingthrough onaction items.

  • Act as a liaison between the requestor and specialty office.

  • Submitall required documentation for specialty referrals.

  • Identifynew specialists to expand the network of specialty offices available to our patients.

  • Collaborate with the team and take on special projects ortasks asassigned,demonstratingadaptability and a proactive approach

Your Qualifications

  • High School Diploma or equivalent experience.

  • 1+ years call center or related customer service experience.

  • Experience in the healthcare,insuranceor pharmacy industry highly desirable

  • Previousexperience in data entry and administrative tasks.

  • Strong communicationskills, both written and verbal.

  • Detail-oriented with a commitment to accuracy in patient data management.

  • Ability to multitask and prioritize tasks effectively.

  • Familiarity with medical record processes and specialty referrals is a plus.
