Overview
The Permanent Supportive Housing Case Manager will play a crucial role in the State's Permanent Supportive Housing Assistance Medicaid Program. Responsibilities will include assessment and eligibility determination, service authorization, and comprehensive care coordination. This position will be part of a small, dynamic team dedicated to collaborating with community-based service providers to address homelessness in Vermont. This position, Permanent Supportive Housing Case Manager (Job Requisition #54156), is open to all State employees and external applicants. Please note that multiple positions in the same work location may be filled from this job posting. AHS BACKGROUND CHECKS: Candidates must pass any level of background investigation applicable to the position. In accordance with AHS Policy 4.02, Hiring Standards, Vermont and/or national criminal record checks, as well as DMV and adult and child abuse registry checks, as appropriate to the position under recruitment, will be conducted on candidates, with the exception of those who are current classified state employees seeking transfer, promotion or demotion into an AHS classified position or are persons exercising re-employment (RIF) rights. Environmental Factors
Work is performed primarily in an office setting and in a variety of field situations. Private means of transportation should be available since travel to field sites will be required. Minimum Qualifications
Master's degree in education, human services, health, counseling or related field. OR Bachelor's degree in education, human services, health, counseling or related field and one (1) year of experience or training in housing, human services, mental health, substance use, public health, education, counseling or related fields. Preferred Qualifications
Experience with community-based health/behavioral health and/or homeless assistance/prevention programs. Special Requirements
