SUMMARY: Provides initial comprehensive assessments for appropriate interventions and measures the effectiveness of treatment provided. The Case Manager will provide one case management session with each participant weekly during their 3-month home-based Cardiac Rehabilitation (CR) intervention. This will include patient counseling, exercise training, and exercise prescription. The Case Manager will also be available via secure portal messaging for questions about integrating home-based exercise into their maintenance routines. Develops protocols to carry out established research objectives. Participate in ongoing evaluation of study protocol. Suggest improvements to data control instruments and measurement instruments where appropriate. Reports to Project Director problems encountered in performing assigned activities suggesting solutions to those problems. Reports to the Principal Investigator and the Study Cardiologist when there is a serious adverse event or a medical concern. Works with the Project Director to maximize the ease of the study administration and follow-up. Maintains quality assurance and safety control in accordance with established hospital department policies and objectives. Collects data on all patients and incorporates findings into the patient's individualized treatment. Evaluates symptomatic responses to therapy and assesses appropriateness of responses to the prescribed intensity. Refers to additional medical or nursing resources when needed. Provide stress management strategies such as relaxation, meditation, and yoga when appropriate. Provide individualized nutritional strategies for healthier eating. Performs 30-day reassessments, evaluates progress towards established goals and identifies any additional target interventions or education. Obtains Principal Investigator's consent on all treatment plans & assessments Need for schedule flexibility to accommodate participant visits across time zones (some early mornings, evening and weekend availability may be needed). In conjunction with the research team, considers risk stratification and comorbidities of patient population within the caseload for optimal care delivery and targets psychosocial, nutritional, tobacco sensation and exercise interventions based on evidence-based practice guidelines. Performs other duties as assigned. MINIMUM QUALIFICATIONS: Bachelor's or master's degree in nursing or Exercise Physiology with experience working in Cardiac Rehabilitation. Knowledge of research grants administration and current reporting requirements preferred via human clinical research experience (during education or after training). EXPERIENCE: Five (5) years of clinical experience in a Rehabilitation environment is strongly preferred. Current CPR and ACLS certifications required. ACLS can be obtained withing the first 3-6 months. ACSM and AACVPR certifications preferred. Supervisory Responsibility: None.
