Job Description

The Social Worker will support the Transitions of Autism Care (TRAC) Project, a multi-year project focused on improving transitions from child to adult systems for young adults with autism spectrum disorder. This role will address the psychosocial aspects of transition by supporting youth and families in navigating complex systems as they move from pediatric to adult care. The Social Worker will serve as a vital team member alongside medical, nursing, and community health staff and will help integrate trauma-informed, person-centered, and developmentally appropriate care into transition planning.

Job Responsibility




  • Conduct comprehensive psychosocial assessments to identify unmet social needs, insurance issues, connections with the appropriate office of disability services, education and employment systems
  • Provide orientation and support to patients and families newly enrolled in the transition consult service as a part of the TRAC project
  • Maintain accurate, timely documentation of initial contacts with patients/families and of psychosocial assessments in accordance with institutional and regulatory standards
  • Provide short-term counseling and prepare comprehensive care plans that support patients and families in preparing for transition to adult healthcare.
  • Provide social work services in accordance with required professional standards, ethical guidelines and best practices
  • Proactively identify and respond to potential care coordination needs by supporting patients and families early in the transition process, and document all case activities, interventions, and outcomes in a clear and organized manner
  • Participate in weekly interdisciplinary team meetings and collaborate with internal stakeholders to design and refine plans supporting this project
  • Support and co-facilitate patient and family engagement activities in accordance with the TRAC project
  • Leverage knowledge of community-based resources to coordinate referrals and establish linkages with community-based organizations to support this project
  • Develop and support psychoeducational programming, including group sessions and caregiver workshops, focused on transition readiness, system navigation, and self-advocacy
  • Promote the TRAC NYC consult service within Northwell and among community partners to enhance awareness and increase appropriate referrals
  • Contribute to program evaluation, quality improvement initiatives, and grant reporting related to psychosocial service delivery and outcomes

Job Qualification



  • Master's degree in Social Work, required.
  • LMSW is required within one (1) year of employment.
  • Personal or professional experience working with individuals with disabilities, preferred.
