The Colorado Coalition for the Homeless seeks a Behavioral Health Clinician I for the PATH team within the Department of Outreach and Engagement Services. The PATH team provides behavioral health services to single adults and couples who are actively experiencing homelessness or are at-risk of homelessness and have a severe and persistent mental illness. The Behavioral Health Clinician I provides therapeutic interventions, assessments and related diagnoses, consultation, education, and referral/linkage for individuals with behavioral health concerns in accordance with the goals of the PATH program and the overall mission, values, and objectives of CCH.
NOTE: In order to ensure that our consumers receive the best possible care the candidate chosen to fill this position will be required to complete our internal credentialing and privileging process prior to a start date. The credentialing and privileging timeline is dependent on the selected candidate's submission of documents necessary for clearance. Please note that a start date will be scheduled once credentialing and privileging is complete.

Essential Job Functions
    • Conduct comprehensive evaluations to identify behavioral health needs, including mental health and substance use issues.
    • Use evidence-based tools for accurate diagnosis and treatment planning
    • Deliver evidence-based individual and group therapy tailored to client needs, focusing on mental health and substance use recovery
    • Create, monitor, and update individualized treatment plans with client-centered goals and measurable outcomes
    • Provide support and intervention during crises, stabilizing clients and connecting them with additional resources as needed
    • Work as part of an interdisciplinary team to deliver coordinated care.
    • Maintain accurate and timely client records, treatment plans, and progress notes in compliance with HIPAA and regulatory standards
    • Help clients access community resources (housing, employment, education) that support their wellness and recovery goals
    • Participate in quality improvement initiatives and stay updated on best practices in behavioral health through professional development sessions
    • Ensure timely & accurate recording of all clinical services, communications, and tasks in the EHR while maintaining complete up-to-date charts using the system and any future upgrades as directed.
Qualifications Summary
    • Master's degree in social work, counseling or similar field.
    • Licensed as a LCSW or LPC preferred, with a CAC II or higher strongly preferred. If not licensed, must already be registered with DORA as a candidate for licensure (i.e. LSW,LPCC) and within 6 months of obtaining LCSW or LPC license.
    • Experience interacting with patients in integrated health care, mental health care, or substance use treatment settings preferred.
$70,176 - $80,702 a year
WHERE A CANDIDATE IS PLACED IN THE COMPENSATION RANGE DEPENDS ON TOTAL RELEVENT YEARS OF EXPERIENCE
Qualified candidates are eligible for the National Health Service Corps (NHSC) Loan Repayment Program . Licensed health care providers may earn funds towards student loans in exchange for a two-year commitment at an NHSC-approved site through this program.
