We Are Inspired to Serve. Join us!The Palliative RN, Case Manager plays a vital role in providing comprehensive and compassionate care to patients facing serious illnesses. This position requires a registered nurse with strong clinical skills, excellent communication, and the ability to navigate complex medical and social situations. The Case Manager collaborates with a multidisciplinary team to assess patient needs, develop and implement individualized care plans, coordinate services, and ensure optimal quality of life for patients and their families.
ESSENTIAL RESPONSIBILITIES:
- Completes an initial assessment of patient and family to determine Palliative Care needs. Provides a complete physical assessment and history of current and previous illnesses.
- Regularly re-evaluates patient nursing needs.
- Initiates and oversees the plan of care and makes necessary revisions as patient status and needs change. Communicates updates as necessary.
- Uses health assessment data to determine nursing diagnosis
- Develops a careplan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventative, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
- Initiates appropriate preventative and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.
- Counsels the patient and family in meeting nursing and related needs.
- Provides health care instructions to the patient as appropriate per assessment and plan of care
EDUCATION AND WORK EXPERIENCE:
- Required Degree: Associate's Degree in Nursing
- Preferred Degree: Bachelor's Degree in Nursing
- Certificate(s): RN License and CPR Certification, if required by state
- Experience: 2+ Years
KNOWLEDGE, SKILLS, AND ABILITY:
- Knowledge of home health & Hospice practices, policies and procedures.
- Knowledge of palliative care and ability to provide treatment, care and support for clients living with a life limiting illness.
- Ability to communicate effectively with residents and healthcare professionals.
- Ability to perform thorough and accurate physical assessments to identify patient needs and health issues.
- Knowledge of privacy regulations and maintaining patient confidentiality.
- Ability to analyze complex patient data and clinical information to make informed decisions.
PHYSICAL REQUIREMENTS:
- Ability to stand or sit for prolonged periods of time.
- Ability to lift, carry, push, and/or pull light to moderate amounts of weight up to 50 lbs.
- Ability to operate various equipment based on department and weather conditions.
- The ability to hear, understand, and distinguish speech and/or other sounds (e.g., machinery alarms, medical codes or alarms. Four hearing requirements listed: In person speech, Telephone, Other remote speech and other sounds.
- Clarity of Vision:
- For near visual acuity Clarity of vision at approximately 20 inches or less (i.e., working with small objects or reading small print), including use of computers.
- For far visual acuity, Clarity of vision at 20 feet or more. This is not just the ability to see a person or object, but the ability to recognize features as well.
- For peripheral vision, observing an area that can be seen up and down or to right or left while eyes are fixed on a given point.
$38.17 - $48.10 per hour
Reasonable Pay Estimate A reasonable estimate of the pay range for this position is $38.17 - $48.10 per hour. There are numerous factors taken into consideration in determining the actual offered rate of pay, including but not limited to: job-related qualifications, experience, skills, education, geographic location, and consideration of internal and external equity.
