Job Description:The Complex Care Coordinator supports the Behavioral Health Group Treatment Program, Partial Hospitalization and Intensive Outpatient levels of care by facilitating and supporting patient discharges to community-based services and higher levels of care, increasing family engagement with patients, and ensuring a smooth transition throughout the treatment process. Responsibilities include coordinating discharge referrals, scheduling outpatient appointments, providing post-discharge follow-up, facilitating psychotherapy groups, supporting the therapeutic milieu, completing suicide risk assessments, and performing administrative duties as assigned by department leadership. Education/Experience/Skills Requirements: Bachelor's degree in social work, Psychology, or a related field - Required Experience: 1 year of experience in Behavioral Health (Inpatient, residential treatment, or outpatient treatment) - Preferred Certifications/Licensures: Basic Life Support (BLS) - American Heart Association - Required CPI (Crisis Prevention Institute) Training - John Muir Health - Required within 30 days of hire; renewed biannually (every 2 years) Skills: Proficiency in computer use, including word processing, internet navigation, and email communication - Required Work Shift:08.0 - 08:00 - 16:30 No Waive (United States of America) Pay Range: $37.30 - $51.63 Hourly Offer amounts are based on demonstrated/relevant experience and/or licensure. Pay will be adjusted to the local market if hired outside of the Bay Area. Note: Positions at JMH which are exempt (not eligible for overtime) under the level of Manager are listed as hourly for compensation purposes on this posting.
