Position Summary Columbia University Irving Medical Center (CUIMC) is seeking a Registered Nurse I to join our ColumbiaDoctors Primary Care team. The Registered Nurse I will report directly to the Director of Nursing. This ambulatory nursing position will serve as a key member of the patient care team and will support a pediatric and adult population across 3 primary care practices. The Registered Nurse performs telephonic, electronic, and in-person nursing assessments consistent with the NYS nursing scope of practice. The role serves as a clinical lead in the practice with responsibility for the maintenance of infection prevention and control procedures in addition to precepting new clinical support staff. Responsibilities Administrative/Operations
- Executes nursing interventions by collaborating with the care team and following the standards of care established by Columbia Primary Care
- Assesses and systematically collects data related to patient health status and works with the care team to develop the plan of care
- Utilizes data to continuously meet care coordination needs across settings in conjunction with the patient, family, and/or other care providers
- Assumes responsibility and accountability for effectively managing nursing care of individual patients
- Documents nursing assessment and interventions to reflect a comprehensive and integrated approach to patient care
- Identifies learning needs of the patient and families, educates by adapting to patient and family learning style, utilizing the teach-back method
- Provides care that is safe, ethical, patient- and family-centered, culturally congruent, and evidence-based
- There are skills and responsibilities that are common across Columbia Primary Care. The Registered Nurse (RN) may perform any or all of these skills and responsibilities:
- Telephone, My Chart, and in-person triage (for symptom management) under supervision as appropriate
- Review and act on test results (lab, x-ray)
- Review and manage communications from internal and external customers, inclusive of in-basket messages, updating immunizations, and updating health maintenance items.
- Participate in providing patient education through multiple avenues, inclusive of in-person, by telephone, or video
- Execute delegation protocols
- Assist with medication management (e.g., medication renewals), including medication prior authorizations as needed. Administer immunizations as an adjunct to the LPN staff
- Complete, accurate, and timely documentation in the electronic medical record
- Assist with the management of urgent and emergent patient care situations
- Assist with the coordination of care across the continuum (e.g., inpatient, outpatient, home, skilled nursing facility, schools)
- Assist with the management of patients with multiple chronic health conditions, acute episodes of care, behavioral health challenges while promoting and supporting prevention and wellness. Perform patient outreach related to gaps in care.
- Monitor supply inventories for practice and order supplies as needed. Assists the Supervisor and/or Manager with re-orders and inventory tracking.
- May float to other clinics as needed for business continuity. Occasionally, provide assistance in the clinical duties of MA and LPN staff in the interest of patient care
- Serve on relevant committees, task forces, and workgroups within Columbia Primary Care and the Organization
- Other duties as assigned, appropriate to the scope, education, and/or training
People
- Precept clinical staff (primarily Certified Medical Assistants) and perform competency testing for new and established staff as appropriate.
Compliance
- Maintain patient privacy and confidentiality according to HIPAA requirements at all times.
- Keep current on all organizational and practice policies (e.g., infection control, HIPAA), goals, initiatives, and required trainings
- Ensure that University and Department policies and procedures are followed.
- Successfully complete all required university, hospital, and department trainings.
- Other duties as assigned within the professional scope of practice/training/education.
Minimum Qualifications
- Bachelor of Science degree in Nursing or equivalent of education, training, and experience.
- An equivalent combination of education, training, and experience may be considered.
- Current and unrestricted New York State RN license.
- Basic Life Support certified.
- Ability to work indoors in an environmentally controlled environment with a degree of dust, noise, odors, etc., without undue discomfort.
- Ability to perform sedentary work that primarily involves sitting or standing for long periods.
- Travel between clinical sites may be required depending on operational need.
- Duties performed routinely require exposure to blood, bodily fluids, and tissue. The incumbent is expected to perform and/or/assist in emergency care and/or first aid as needed.
- Ability to work collaboratively with a culturally diverse staff and patient/family population, demonstrating tact, respect, and sensitivity.
- Must have excellent communication skills and the ability to communicate effectively with a diverse audience of peers, management, and other stakeholders. Must have highly developed interpersonal skills and the ability to de-escalate stressful situations.
- Must be an engaged and positive team member demonstrating collaboration, inclusion, reliability, punctuality, and adaptability to new and changing situations, including helping peers adapt to new situations.
- Strong proficiency in computer skills including, but not limited to, Electronic Health Systems, Microsoft Excel, Microsoft Word, Email systems, etc.
- Must be a motivated individual with a positive mindset and exceptional work ethic.
- Must successfully complete systems training requirements.
Preferred Qualifications
- Physician practice experience preferred.
