Community Health Worker
Overview Presbyterian is hiring a Community Health Worker (CHW), to work under indirect supervision, provide patient support in the ambulatory care setting according to established standards of care in collaboration with Care Managers, Medical Providers, and other members of the health care team. In addition, connecting patient to community-based programs to improve their health and general well-being through education and provision of coordination of care and services. This CHW role will support pregnant populations in an inpatient care setting, who have been exposed prior to birth to a controlled substance, illicit substance, or alcohol. The CHW staff member assigned to this workflow and targeted population will serve as fully participating members of the care team, will support patients meeting CARA criteria through coordination with managed care organizations, and through providing resources for social drivers of health (SDOH) and other health-related social needs (HSRNs).
