Position Summary

The Step-Down Navigator is responsible for meeting with all eligible patients to discuss the benefits of stepdown services as part of their continuum of care treatment. The Step-Down Navigator will be responsible for communicating eligibility to key members of senior leadership & associated departments and will be responsible for routinely collecting and entering data, specific to eligibility, into Midas for trending report purposes.

Job Duties/Responsibilities

• Coordinates with the Admissions Department, Social Services, Utilization Management, the Business Office and Outpatient Services on patient eligibility for programming options.
• Ensures that all aspects of the step down process are followed: identification of eligible patients, insurance verification, treatment team approval etc.
• Meets with 100% of eligible patients to discuss the benefits of appropriate programming options.
• Conducts patient discharge groups.

• High School Diploma required. Bachelor's in social services or patient care related field preferred.
• Current BLS, Physical and Verbal Crisis Intervention Certification required (Training may be obtained during new hire orientation).
• One (1) year of clinical experience in an inpatient psychiatric setting required.
• Thorough understanding of the Inpatient workflow process, as it pertains to case management and discharge planning.
• Good oral and written communication skills required to communicate in a clear and concise manner with patients, physicians, staff and administration.
