The Transplant Coordinator is responsible for managing nursing/clinical care in a transplant program setting. The position carries out direct and indirect patient care responsibilities in collaboration with the Transplant Team with emphasis on independent decision-making and leadership skills. The position demonstrates the ability to provide consistent direction for designated nursing/clinical personnel in planning, organizing, implementing and evaluating nursing/clinical activities; collaborates with the Transplant Team in promoting customer focused delivery of service, compliance with and maintenance of all regulatory standards, hospital and team policies/procedures and quality/patient safety standards, and achieving departmental and organizational initiatives in a fiscally responsible manner; and utilizes independent judgment and knowledge of clinical operations to provide a direct interface with essential partners (e.g. Ambulatory Clinics, Inpatient departments, Lab, Pharmacy, etc.). Demonstrates knowledge of program specific responsibilities for care delivery, organizational skills, to include prioritization and time management of clinical flow in both inpatient and outpatient settings. Develops, implements, and monitors care coordination plan of care for transplant patients and families from evaluation, through candidacy, transplant, and post-transplant. Develops and performs transplant education for patients and families that is appropriate for patients'/families' age, developmental stage, primary language, etc. Coordinates donor searches/offers. Includes on-call responsibilities for solid organ transplant coordinators. Participates in transplant consent process in providing patients/families education and being a resource for information. Develops, implements, and documents care coordination in compliance with regulatory requirements. Develops individual goals and initiates own evaluation within department to enhance clinical practice and professional growth by assuming a leadership role in clinical education and the evaluation and communication of committee goals and outcomes. Utilizes evidence-based practice to direct the development, revision and evaluation of policies, procedures and guidelines for practice. Encourages clinically based research projects to evaluate practice outcomes. Evaluates effectiveness of committee structure, purpose, and outcomes within and across disciplines. Mentors staff in creating/evaluating projects focused on improving clinical practice and recommending changes. Utilizes expert evidence-based practice to provide clinical expertise to the healthcare team in coordinating developmentally appropriate care and evaluating outcomes for infants and children with complex health care needs. Provides clinical follow up when desired outcomes are not achieved. Establishes Clinical Care Guidelines & Nursing Triage Guidelines/Standing Orders in order to track and ensure evidenced based outcomes. Educates healthcare team on the management of specific patient population needs. Minimum Qualifications EDUCATION - Bachelor of Science, Nursing from an accredited college or university. EXPERIENCE - Three years clinical experience required. CERTIFICATION(S) - Basic Life Support (BLS) for healthcare providers/CPR, certification required. LICENSURE - Registered Nurse licensure is required.
