POSITION SPECIFIC SUMMARY: Serve as a clinical pharmacist on a large, multisite research project, working remotely with enrolled study subjects located in Illinois and their health care providers to improve their cardiovascular disease risk. This is a part-time position 25% up to 50% FTE. Key Areas of Responsibilities and Specific Job Duties and Tasks Documents All Patient Care Activities Performed The clinical pharmacist provides pharmacy-related services through sharing of information with other members of the healthcare team via documentation within the patient medical record and other on-line reporting systems. Patient drug allergies and patient-specific height and weight information. Medication histories. Medication counseling to patients. Interventions with prescribers and other healthcare providers. Patient interactions - those that occur in the clinic and when the patient is located elsewhere (e.g., at home, as an inpatient, in the Ambulatory Care Pharmacy). Drug dosing recommendations made to prescribers. Drug Use Evaluation Program activities. Steps taken to identify, evaluate, or avoid adverse drug reactions. Pharmaceutical Care Operations Provide clinical pharmacy services or other specialized pharmacy services to specialized patient populations to optimize patient outcomes and safety. Provide expertise in developing systems of care. Analyze and improve medication usage. Identify and implement best practices into guidelines and policies. May participate in IRB or P&T committees. Evaluate patient and drug-related data to identify drug therapy problems and formulate an appropriate plan of care. Assess patients, including identifying and prioritizing patient problems and medication-related needs. Evaluate drug therapy for appropriateness, effectiveness, safety, adherence, and affordability. Follow up on and monitor the outcomes of therapeutic plans. Demonstrate and apply in-depth knowledge of pharmacology, pharmacotherapy, pathophysiology, and the clinical signs and symptoms of diseases and/or disorders. Uses organizational skills to identify and apply tools and resources to efficiently and effectively work independently. Experience providing chronic care management. Education Requirement A Pharm.D. Desirable Qualifications Completion of an ASHP accredited PGY1 Residency. Licensure as a Registered Pharmacy by the Illinois Board of Pharmacy Previous experience conducting motivational interviewing Previous experience providing remote, telehealth to manage chronic disease states.
