This position will be hybrid - M-F 730-4, 2 days in the office. This is also more of an administrative position.

Summary:

Working under the supervision of a licensed professional, the Population Health Specialist is responsible for managing operational and administrative processes for population health and care coordination programs that include, but are not limited to, Transitional Care Management (TCM), Complex Care Management (CCM), Remote Patient Monitoring (RPM), Mobile Integrated Health (MIH) and care gap closure. The CMA interfaces with and acts as a resource to team members, other Grady Health System departments, patients/caregivers and physicians/care providers, and external partners including home health agencies, transportation vendors, durable medical equipment/medical supplies providers and community service organizations.

Qualifications:

* High School Diploma or GED. At least 2 years of college course work preferred.

* Medical assistant certification (example CMA/RMA).

* Three or more years of experience in a health care or community resources setting.

* Experience with EMR and computerized charting, preferably in Epic EMR.

* Case management experience preferred.

* Excellent customer service skills.

* Local knowledge about and connections to community health care and social resources

* Ability to multitask.

* Ability to speak a second language, preferably Spanish.
