SCDI Support Coordinator, OBGYN, MFM

The Population Health Coordinator (PHC) works with the Senior Population Health Coordinator and assigned
practice teams to ensure that patients receive the condition-specific services they need. For example, PHCs will:
coordinate proactive patient outreach to schedule follow up appointments, screen/monitor patients using questionnaires
regarding mental health or other conditions, coach patients as they prepare for cancer screening procedures, and provide
reminders about their pre-visit lab tests.

PRINCIPAL DUTIES AND RESPONSIBILITIES:
1. Gather and manage quantitative and qualitative patient data using EHR population registries and evidenced-based assessment tools
2. Attend practice and central team meetings as directed
3. Provide regular updates and reports to the Division Director, Medical Directors and Practice Manager
4. Creates and manages patient registry reports required to meet the needs of the members of health care teams at assigned practices.
5. Provide data support (when applicable) for practice staff in the development of creative processes to proactively manage target populations
6. Learn and understand OB/GYN practice workflows with respect to optimal and coordinated health care for target patient populations
7. Data gathering: conducts ad hoc analyses as needed for each practice. Gathers and sorts required data and generate pre-established reports. Recognizes and reports data inconsistencies to appropriate personnel
8. Performs all job functions in compliance with applicable federal, state, local and company policies and procedures
9. Conduct timely outreach tailored to meet each patient's condition specific needs
10. Work as an effective member of the team and serves locally as a representative within a practice's interdisciplinary team
11. Follow established communication protocols and informs clinical team and program leadership accordingly
12. Performs all other duties, as required

Qualifications:

  • High school diploma required. Bachelor's degree in social sciences or health management preferred
  • At least 2 years of prior healthcare experience required
  • Proficiency in medical terminology and HEDIS quality performance knowledge preferred
  • Patient-focused experience or customer service background strongly preferred
  • Working knowledge or familiarity with electronic health records and other health care IT systems desirable

SKILLS/ ABILITIES/ COMPETENCIES REQUIRED:
* Ability to work independently in a busy open office setting with other employees
* Proficiency in MS Word, Excel, Access and PowerPoint
* Excellent organizational skills and ability to prioritize a variety of tasks
* Careful attention to detail
* Excellent interpersonal skills are required
* Good oral and written communication skills
* Ability to learn new skills and procedures quickly and perform them independently
* Ability to demonstrate professionalism and respect for subject rights and individual needs
* Ability to sensitively engage diverse groups of patients regarding their participation in their healthcare
* Must be able to be HIPAA compliant at all times
