6 years of experience in medical and/or psychiatric social case work to include 1 year as Social Worker I or equivalent Education may be used in lieu of experience

JOB DUTIES
- Interviews patients and occasionally interviews family members to obtain information concerning home environment, family relationships, health history, financial status, and personality traits to better understand personal needs or problems of each patient.
- Confers with medical personnel with respect to plans or programs developed for patients.
- Evaluates data gathered in terms of plan of medical/psychiatric treatment and available community social service programs. Plans pertinent health maintenance or rehabilitation program for each patient, and completes social work evaluations or forms related to their disability.
- Assists patients and their families to understand, accept, and follow medical, psychiatric and/or other recommendations.
- Provides variety of services, such as advising on social problems, arranging for postoperative care at home or in institutions or for admission to nursing homes, alleviating of anxieties concerning permanent disabilities, disfiguring illnesses, uncertain future, etc.
- Refers patients to appropriate community social service agencies for financial assistance, employment, legal aid, or for assistance in resolving other personal or family problems.
- Works with insurance companies to obtain medications, orthotics and/or disability aids.
- Maintains chronological notes, clinic charts, or case histories for each patient with respect to social problems, adjustment of patient and family, and actions taken or planned by social worker.
- Interprets to other social service agencies, medical or psychological condition of patients as related to their emotional adjustment, physical limitations, employment, and financial needs, and to medical staff, those social, economic, or emotional factors which may be related to patient's medical care and physical rehabilitation.
- Plans, coordinates, and supervises, work of assigned professional and paraprofessional social work personnel of lower grade.
- May participate in clinical and laboratory research in such areas as animal research, child development, physiological and behavioral functioning, etc.
- Performs other duties as assigned.
