Performs comprehensive case management intake assessment and develops plans with clients for obtaining services needed. Coordinates essential services to individuals and families seeking services at Community Healthlink.

I. Major Responsibilities:

1. Performs comprehensive case management intake assessment and develops plans with clients for obtaining services needed.

2. Completes initial behavioral health intake documentation as required by the program, payor, and regulatory bodies.

3. Works collaboratively with the integrated team to coordinate the delivery of appropriate behavioral health and other support services, including coordinating supportive transitions in care providers.

4. Provides ongoing case management services to assist clients with obtaining various entitlements and benefits (i.e., SSI (Social Security Insurance), EAEDC (Emergency Aid to Elderly and Children), MassHealth and other health insurance needs, addiction services, and other community resources).

5. Provides transportation assistance to individuals as needed. Assist clients in accessing transportation through existing resources or accessing new resources.

II. Position Qualifications:

License/Certification/Education:

Required:

Case Manager-I- High school diploma/GED.

Case Manager-II - Bachelor's degree.

Satisfactory CORI background check.

Preferred:

1. Associate's degree.

2. Bachelor's degree in psychology or human services or a closely related field is preferred.

Experience/Skills:

Required:

1. Ability to develop a working knowledge of CHL's service lines in order to appropriately triage individuals being assessed for services.

2. Excellent oral communication and computer skills

3. Ability to multitask. Must possess time management skills with a strong sense of prioritization and follow through.

4. Requires personal and professional accountability, self-management, and initiative. Professional integrity and cultural competency.

5. Ability to use office equipment, including copy machines, computers, printers, telephones.

6. Ability to organize complex tasks and meet deadlines.

Preferred:

1. Prior experience in a behavioral health or social services setting is preferred.

2. Basic knowledge of CHL services and insurance requirements and reimbursement for these services

3. Working knowledge of Electronic Health Records (EHR)

Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.

Department-specific competencies and their measurements will be developed and maintained in the individual departments. The competencies will be maintained and attached to the departmental job description. Responsible managers will review competencies with position incumbents.

This position also includes engagement work with clients to troubleshoot barriers to attending treatment and to support them in receiving care within or outside of CHL.
