MINIMUM QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. Masters Social Work Degree required

2. LSW/LCSW certification required in the state of West Virginia, or the state where services will be provided

EXPERIENCE:

1. 5 years of clinical hospital experience required

2. 2 years of Care Management experience required

OR

MINIMUM QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. Bachelor's Degree Required or obtained within 5 years of hire (for all candidates hired after October 1, 2017)

2. Current unencumbered licensure with the WV Board of Registered Professional Nurses, or appropriate state board where services will be provided, as a registered professional nurse OR current multi-state licensure through the enhanced Nurse Licensure Compact (eNLC).

EXPERIENCE:

1. 5 years of clinical hospital experience required

2. 2 years of Care Management experience required

PREFERRED QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1.Certification in Care Management preferred

CORE DUTIES AND RESPONSIBILITIES:The statements described here are intended to describe the general nature of work being performed by people assigned to this position.They are not intended to be constructed as an all-inclusive list of all responsibilities and duties.Other duties may be assigned.

1. Develops and implements department specific staff educational activities.

2. Collaborates with department leadership and support staff using indicators, such as, regulatory agencies, new policy development, new technology, patient case study and quality assurance monitors, to prioritize educational needs.

3. Assist the department leadership in the assessment of new departmental employee learning needs and development of employee orientation and education plan to ensure consistency in implementation validation and documentation methodologies.

4. Participate in weekly meetings with new employees and Supervisor/Manager during orientation phase to evaluate the staff progress and adjust plans accordingly.

5. Performs 45 and 90 day performance reviews in conjunction with Supervisor/Manager.

6. Demonstrates clinical expertise by serving as a resource and role model for department personnel.

7. Facilitates the orientation, staff development, continuing education, competency validation, patient education and academic experiences.

8. Defines and coordinates orientation/ on-boarding needs for all new department employees, as well as accepted students.Individualizes orientation, with mentor, evaluates orientee's progress, and provides ongoing constructive feedback to both orientee, their mentor and the Supervisor/Manager.

9. Assists departmental executive leadership team in the development of annual competency analysis system requirements and criteria to assure knowledge, skills and attitudes that are necessary to fulfill role expectations in the institution.

10. Provide constructive and timely feedback to Supervisor/Manager on employee performance and management.

11. Supports an environment for innovation, creativity and empowerment with care delivery based on current literature and research.

12. Demonstrates regard for the dignity, respect and safety of all patients, significant others, visitors and hospital personnel as defined in the philosophy of the organization.

13. Promotes and contributes positively to intradepartmental and interdepartmental relationships.

14. Contributes positively to the teamwork of the department by assisting other staff, contributing ideas and helping to resolve problems

15. Serves as role model and promotes within the staff effective communication and conflict resolution skills, reflecting quality service attitudes and keeping consumer satisfaction in mind.

16. Performs monthly compliance audits as directed

17. Follow yearly license renewal for compliance

18. Serves on various hospital committees as needed or assigned.

19. Educates hospital staff and physicians to payer regulations and managed care principals to ensure seamless patient care.

20. Fosters the integration of staff and/or students into the healthcare team

21. Improves and maintains management skills by participating in self-development activities such as seminars, workshops, classes and literature review.
