The Project Access-New Haven (PA-NH) Health Advocate reports to the Program Supervisor and works closely with the Yale University research team, patient care teams, and other PA-NH staff. In collaboration with the team, the health advocate actively engages with and coordinates the recruitment, enrollment and follow-up for participants who require assistance managing their blood pressure and enroll in PRESSURE CHECK, a multi-year study, carried out at Yale New Haven Hospital and community settings. The study seeks to improve the quality of health care offered and to help prevent/manage hypertension outcomes following community-engaged approaches. Qualifications EDUCATION Associate or Bachelor's Degree in a related field preferred, or an equivalent combination of education and experience. EXPERIENCE (3) years or experience with a High School Degree, (2) years of experience with an Associate Degree or (1) year of experience with a Bachelor's Degree.
