Responsibilities

Intensive Outpatient Program

Provides clinical care and management of a caseload of patients, many of which are in a sub-acute phase of treatment preceded by an inpatient stay or emergency room visit. Referrals are also obtained from community providers for those needing a higher level of care and self-referred individuals. Practice within collaborative interdisciplinary team of providers (MD, APRN) and support staff to provide a group therapy utilizing evidence-based modalities. Individual, family, and crisis interventions are provided by the clinician as needed. Provide discharge planning for continuity of care and case management connecting patients to community resources.

  • Participates in diagnostic assessment of patient at intake and ongoing assessment throughout duration of program.
  • Conducts and provides timely documentation of biopsychosocial assessment, group therapy, individual sessions, crisis evaluation, and discharge summary in appropriate records including the electronic medical record of patient status/progress.
  • Participates in team meetings and shares assessment and treatment information with other team members through appropriate, timely and clear communication.
  • Work collaboratively with primary care practice Physicians, Medical Assistants and Nurses in the various treatment and practice settings as needed.
  • Provides appropriate individual, family, couples and group psychotherapy to patients based on an interdisciplinary treatment plan.
  • 6. Participates in the formulation and implementation of an individualized interdisciplinary treatment plan based on a thorough assessment of the patient's problems and needs.
  • 7. Assists psychiatric medical staff in monitoring the use, effectiveness, and side-effects of psychiatric medications.
  • 8. Demonstrates basic clinical and technical skills according to established standards of practice, including implementation of evidence-based interventions.
  • 9. Actively participates in patient and family/significant other(s) education.
  • 10. Demonstrates the ability to establish and maintain meaningful therapeutic relationships with patients.
  • 11. Effectively manages crisis situations/routine treatment interactions with respect for patient rights, legal/ethical obligations, and therapeutic goals of any interventions utilized.
  • 12. Conducts and documents regular reviews of treatment effectiveness/patient progress.
  • 13. Provide discharge planning, including contacting mental health agencies and referring patients for case management, residential, and community based follow up services.
  • 14. Contacts treatment providers involved in patient care and obtains collateral data as well as providing continuity of care information of patients as needed.
  • 15. Meets performance expectations for Customer Service, Teamwork, Resource Utilization, and Staff and Self Development as outlined in the performance review.
  • 16. Performs other duties as assigned or directed to ensure the smooth operation of the department/unit.

Qualifications

EDUCATION

MSW, MA, MS, MFT

EXPERIENCE

Two (2) years experience preferred, will consider well qualified candidates if less than 2 years practice experience

LICENSURE

Licensed as one of the following: LCSW, LMFT, LPC, or LMSW

SPECIAL SKILLS

Knowledge of clinical measures such as PHQ-9, GAD-7, CSSRS, etc. and ability to administer to patients to assess progress and patient outcomes. Knowledge of common safety hazards and precautions to establish/maintain a safe work environment. Knowledge of health promotion, health risk identification, and patient education principles and techniques. Develop and use skills in the implementation and use of the electronic medical record (EMR) applications. Ability to analyze complex data and draw conclusions. Ability to interpret, adapt, and apply guidelines and procedures. Ability to establish/maintain effective working relationships with patients, clinical staff, and community agencies. Population expected to work with currently include. Adults 19- 64 and Geriatrics 65 + years of age. In the future program could expand to Pediatric and Adolescent populations.
