Overview

Responsible for the day-to-day management of clinical programs including daily scheduling, coordination and planning. Performs intake and follow up assessments for clinic patients with standardized templates. Other duties listed in this description as well as those assigned as needed.

Responsibilities

  • 1. Manages the day to day operations of the assigned Clinical Program.
    • 1.1 Develops, monitors, and maintains schedules in a timely fashion.
  • 2. Maintains and monitors departmental patient records and statistical data as mandated by department policies and regulatory agencies requirements.
    • 2.1 Submits statistical reports by established deadline 100% of the time as indicated by monthly log reports. Assist with creating and maintaining evidence-based quality metrics for the Palliative Outpatient Clinic.
  • 3. Participates in professional staff development and hospital/community activities to maintain professional competency, contribute to training of others, and to enhance professional visibility.
    • 3.1 Attends and participates in 90-95% of departmental meetings and staff development programs as measured by review of meeting minutes and by supervisory observation.
  • 4. Participates in programs, policies and procedures development and implementation.
    • 4.1 Identifies problem areas or gaps in services in hospital and department in accordance with professional standards measured by supervisory observation.
  • 5. Maintains effective, open communication with all members of the health care team and other hospital staff.
    • 5.1 Functions as an active, contributing member of an inter-disciplinary team as measured by attendance at team meetings, supervisory observation and feedback.
  • 6. Develops and maintains positive working relationships with community agencies/facilities and service providers to enhance the availability of resources and to facilitate patient access.
    • 6.1 Maintains current information about resources as measured by supervisory observation.

Qualifications

EDUCATION

Registered Nurse, BSN required

EXPERIENCE

5 years experience preferred

LICENSURE

Registered Nurse-CT, Basic Life Support (BLS)

SPECIAL SKILLS

Supervises work. Is a team leader. Oversees documentation and performance improvement initiatives. Assists Director with staffing, budgeting compliance and providing staff educational needs. Demonstrates service excellence and is a role model for staff

PHYSICAL DEMAND

While performing the duties of this job, the employee is regularly required to stand, walk, and talk and/or hear. The employee is also required to sit. The noise level in the work environment is usually moderate.
