About this job:

This is an exciting opportunity within the growing Pediatric Complex Care Collaborative (PC3) program at the Johns Hopkins Children's Center. The PC3 program aims to improve the lives of our most complex patients and their families through collaboration and coordination across the care continuum, with an emphasis on keeping the patient at home whenever possible.

In partnership with our multi-disciplinary team, the RN Case Manager exclusively serves the PC3 population, with work focused on aligning and coordinating care based on the patient and family needs and connecting patients and families with appropriate resources in the Children's Center and community. The RN Case Manager works with entire PC3 team to coordinate and streamline patient/family experience, following complex cases across hospital settings and the care continuum. The RN Case Manager will also serve as a primary point of contact for patients/families to the program, employing nursing processes to obtain objective and subjective data about the patient, responding to patient calls and MyChart messages requiring triage of urgent symptoms, and providing patient education for acute and chronic medical conditions.

What you will bring:




  • Completion of an accredited RN nursing program; BSN preferred.


  • Minimum one year of Pediatrics RN experience required; ideal candidate will have 5 years' RN experience.
  • Preferred experience in case management, discharge planning or utilization review.
  • Valid Maryland Nursing License required.
  • Accredited Case Manager - Registered Nurse (ACM - RN) / Certified Case Manager (CCM)preferred



Salary Range: Minimum 39.94 per hour - Maximum 61.91 per hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.

In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
